Ask your references if you can use them. Get a direct line or direct email address. Ask them what they will say about you, so you can determine if you want them to speak about you. Feel free to offer examples of your work habits to jog their memory.
When you accept a position, remember to thank them for their input.

Review questions most often asked at interviews and prepare answers. Practice answering out loud. You don't want the first time you hear the answer to be at the interview. Likely it will sound weak. The more you practice the stronger the conviction.
When they ask if you have any questions, have four to five of them. An internet search will provide you with many, many possibilities.
At the end of the interview, if you want the position, ask for it!! In your own words, let them know you want to be part of their team. If you don't, they may assume you're not interested.
After the interview, always send a thank you note and restate your desire to work for them. Before it was recommended you send a hand-written note; now days it's acceptable to email your thanks. While we're on the subject of thank you notes, be sure to thank anyone who helped you along the way - your references, your job coaches, your network contacts who mentioned openings, anybody.
